    Adult day care centers serve as an emerging provider of transitional care and short-term rehabilitation following hospital discharge. As a preferred platform for chronic disease management, adult day centers are an interactive, safe and secure environment for participants requiring supervised daily care. While nearly half of all participants nationally have some level of dementia, other common chronic diseases among participants include chronic hypertension, physical disability, cardiovascular disease, diabetes, mental illness and developmental disability.
